| Transitions Children's Services (TCS)
was founded in the summer of 2009
by Brian Van Anne, MSW. Mr. Van Anne
brings close to fifteen years of
social work experience to this agency,
eight of which he served as a foster
family agency social worker, foster
family agency supervisor and foster
family agency director for another
local, well respected foster family
agency. Mr. Van Anne has a Bachelor of
Arts degree in Social Work and Master
of Social Work degree from California
State University, Fresno. |

| TCS' purpose is to provide quality, culturally competent treatment foster care services to children and families in the child welfare/foster care system. TCS will partner in philosophy
and delivery of service with
surrounding counties in order to meet
the needs of their consumers. TCS aims
to use a Strength Based approach in
recruiting, training and delivering
foster care services with the
underlying theme being that foster
care is a temporary living arrangement
for children; reunification is the
goal and cornerstone of case planning
which will be supported throughout the
treatment plan process. |

| TCS will offer case management services in the form of home visits, the formulation
of treatment plans, foster parent support services and crisis intervention services.
TCS social workers will be expected to make weekly contact with consumers placed in
our foster homes. |

| TCS
social workers will offer support to
foster parents. TCS social workers
will be available around the clock to
provide crisis intervention services.
This includes the assigned social
worker during normal business hours
and a rotating emergency response cell
phone allowing a TCS representative to
be available to any county personnel,
foster child or foster family
twenty-four hours a day. |

| TCS
social workers will be Masters level
graduates in compliance with CCR
88065.3(b) (1). This level of
education will allow for social
workers to provide therapeutic
intervention during normal and crises
situations. TCS social workers will
assist in the formulation of treatment
goals that will be addressed by the
treatment team (to consist of TCS
social worker, county case manager,
consumer, foster parent and any other
authorized representative). TCS will
also partner with the County and
incorporate their intended services
for the consumer like mental health
counseling, psychiatric assessment and
services, biological family visits and
treatment team staffings. |

| TCS
will recruit, train and certify
perspective foster families under our
State issued facility number. We will use
a comprehensive training model that
addresses CCR regulations (88088(5)(B) and
88069.7(c)(5), including but not limited
to initial and ongoing training in child
abuse identification, prevention and
treatment. TCS will also incorporate
training on Fresno County’s Family to
Family initiative; the core being that
foster families serve as role models and
offer support to families during the
reunification process. |

| Our mission is to serve abused and/or neglected children who have been removed from their homes and are in need of loving, nurturing and competent foster families. To recruit, train and support families interested in fostering children. To provide culturally sensitive services that reflect the goals and objectives of the placing agencies from which we receive referrals. |

| Our social workers will work to meet the needs of any child placed in foster care, support families who open their home to children and work with placing agencies as treatment plans are developed and implemented. |
